2009 Rowan FIRST Lego League Form Download

There are two different forms and one additional list that we need for you to fill out this year.

Please send these to us no later than Tuesday November 24th. Please note that the consent forms for the students require parent or guardian signatures!!

All of the forms can be found at the bottom of this page.

  • One team profile form needs to be filled out per team. Please note that it should include a photo of your team to help our judges remember who you are!
  • A consent form must be filled out by all mentors, team leaders, student participants, and volunteers affiliated with your team. Note that parent or guardian signature is required for all participants under 18!
  • A list of the members of your team

Team Profile Form

Please Fill out one copy of the "Team Profile" form per team. Please note that it should include a photo of your team to help our judges remember who you are! email to Dr. Hong Zhang, zhang@rowan.edu with the subject "FLL-team-profile-form". The form is available for download in 2 formats:

First Lego Consent Form

A consent form must be filled out by all mentors, team leaders, student participants, and volunteers affiliated with your team. Note that parent or guardian signature is required for all participants under 18! The original forms must be presented at registration, but we'd like to have copies of them in advance. We prefer for you to scan your forms and email them to zhang@rowan.edu with the subject "FLL-consent-forms". If you prefer, you may mail copies of your forms to:

IMPORTANT NOTE: You MUST bring the original signed consent forms with you to the tournament. We will collect them from you at the time of check in.

Team Membership List

In a separate email to zhang@rowan.edu with the subject "FLL-team-list", please send us a list of the names of all of the members of your team as you'd like them to appear on certificates of appreciation.
